How to access Support at Home services

If you or a loved one are looking to live independently at home but need some extra assistance, Support at Home services could be the right solution.

This government-subsidised program is designed to help older Australians access the services, equipment, and home modifications they need to remain safe, healthy, and connected to their community.

Step 1: Get Assessed

The first step in accessing Support at Home care services is to undergo an aged care needs assessment. This assessment determines your care needs, eligibility, and the funding level you require.

You can apply for an aged care needs assessment in three ways:

Once your application is received, an assessment organisation will contact you (or a nominated representative) to discuss your care needs and arrange a suitable date for the assessment. Most assessments are conducted in your home but can also be completed at other locations or via telehealth if needed.

The assessor will evaluate your care needs and make recommendations for the type and level of services required. Once approved, you’ll receive a care plan along with a funding classification that determines your Support at Home service level.

Step 2: Income and Assets Assessment for Support at Home

Once your aged care needs assessment is complete, you’ll need an income and assets assessment to determine how much you may contribute towards your care and how much the government will fund. For most people, this assessment is conducted by Services Australia.

After this assessment, you’ll receive a letter detailing your contribution rates for different service categories. Knowing your contribution rates helps you plan your budget and ensures you can access the support you need without unexpected costs.

Step 3: Choosing a Provider

Once approved, you can choose a Support at Home provider to deliver your services. Each provider offers different types of support and care management, so it’s worth comparing options to find a provider that meets your specific needs and preferences.

Support at Home services can include:

  • Clinical care: nursing, physiotherapy, or other allied health services
  • Independence support: personal care, mobility assistance, occupational therapy, respite care, or home modifications
  • Everyday living support: meal preparation, cleaning, gardening, or transport
