Home Care Packages

The Australian Government funds home care packages through the Home Care Package Program (HCP) to provide support for people with moderate to complex needs who would like to remain in their own home. The services are flexible and enable choice and control to support you and your unique needs. You may be eligible for one of four “package” levels, ranging from basic to high-level care needs.

LEVEL 1

Just a little extra help. This may include household or domestic assistance, social support, meals or shopping.

LEVEL 2

Caters for moderate care needs and may include domestic assistance, personal care, social support, meals, shopping or rehabilitation and equipment hire.

LEVEL 3

Available for higher level needs, which may include personal care, household assistance, meals and medications, nursing or allied health input, support with memory or behaviour changes, or assistance with medical aids and appliances.

LEVEL 4

Address complex health care needs including Nursing Care and access to medical aids and equipment in addition to all other support services.

*If your needs change, we will work with you to ensure you receive the right level of support.

How can I access Home Care Packages?

We are here to help. To access Government funded aged care services such as Home Care or Residential Aged Care you must first be assessed by ACAT. You can arrange an assessment by calling My Aged Care on 1800 200 422.

My Aged Care will assess your eligibility for a home care package and assign funds for the appropriate level of support needed. For support and guidance, feel welcome to call one of our Supported Living Coordinators.

Already have a home care package approval letter?

Give us a call. If you’ve already received your approval letter, you can now choose your preferred home care provider. Note, you have 56 days from the date your package has been assigned to find a provider and enter into a Home Care Agreement. If you need an extension and more time to select your provider, please contact My Aged Care 1800 200 422.

Already have a home care package and want to change your provider?

Call Adventist Retirement Plus Supported Living Services – We will arrange a time to visit you to better understand your individual needs and preferences for the care required.
Call My Aged Care 1800 200 422 to let them know that you would like to change your home care provider and ask them for a referral code.
Speak to your current provider – Speak to your current provider and agree on a date when your services with them will end.
Confirm and change – Call our Supported Living Coordinator and confirm the date when your services will end with your current provider. Your services with us will commence from that date forward.

Nothing is too much trouble

“I highly recommend using the CARV Supported Living. Nothing is too much trouble for Jo and the Carers who help Mum with her day-to-day living. She has support with such things as washing, cleaning, showering, shopping and medication supervision, which leaves me to enjoy our time together. If Mum is unwell they are close enough to just pop in to check on her each day and I am able to take holidays with peace of mind that she is being well looked after. As a result Mum is happily living in her own home in the CARV village surrounded by her friends. Thank you to everyone in the Supported Living Unit. You all do an amazing job!”
